Nursing care plan:
Adenocarcinoma is cancer that cancer cells form tissues in the male
productive organ. It usually found on the foreskin of the male
reproductive organ. It has 4 stages. pathophysiology of this cancer
includes HPV infection, being aged above 60 years, poor hygiene,
AIDS, tobacco use, not being circumcised. signs and symptoms of
this cancer include redness, sores, irritation, pain, bleeding,
discharge and lump on the male reproductive organ. complications
include necrosis, edema, urethral fistula, and urethral strictures.
diagnosis and lab tests include biopsy of the tissue, fine needle
aspiration biopsy, X-ray, CT scan, PET scan, ultrasound, MRI to
find the tumor the size and location of the tumor. blood test and
urine test for culture and sensitivity and to check abnormal level
of5-HIAA, chromogranin A test and molecular testing of the
tumor.
NANDA nursing diagnosis includes risk of infection, sexual
dysfunction, impaired physical mobility, Acute pain, altered
urinary elimination, anxiety. Three NANDA priority nursing
diagnosis include Acute pain: Reduce the pain and make the patient
comfortable with excision of the tissue and by continuous
chemotherapy and radiation therapy treatment and administer
analgesics for pain and use relaxation therapy to eliminate pain,
Risk for infection: strict aseptic technique and keeping the
incision area clean and using PPEs to avoid entry of pathogenic
organism into the area and check urine culture for infection.
altered urinary elimination needs continuous monitoring of urine
output, monitor the patient urine frequency, urgency and provide
voiding pattern and maintain hydration status. Evaluate the ongoing
treatment, any complication, and followup.
chemotherapy and radiation therapy in disease management improve
protection intervention from complications and provides long term
outcomes. Medications for penile cancer include 5-FU, efudex, and
imiquimod. it treats the specific area and the use of chemotherapy
anti-cancer drugs to destroy the cancer cells in the body. it
causes side effects like fatigue, nausea, vomiting, alopecia,
diarrhea, skin rashes, etc. It should be taken in 3 to 4 weeks
interval through IV or by oral form. educate the patient to protect
the drug in an enclosed system, ensure safe handling, avoid
contamination and hygiene precautions and avoid alcohol and
smoking. monitor the side effects and inform the doctor if anything
abnormal.
