The patient, a 20-year-old female, presented to the emergency department complaining of bilateral arm and shoulder pain, "yellow eyes," and dark urine. The emergency department evaluation revealed profound jaundice with markedly elevated liver function tests. The patient was admitted for further evaluation. A non-contrast gall bladder ultrasound was negative for gall stones. Hematological studies indicated sickle-cell disease, which could be contributing to the jaundice. Because the liver function gradually improved, it was felt that she could be further evaluated as an outpatient for probable acute hepatitis B.

Answer #1

Sickle cell disease is inherited blood disorder, where blood cells looks like sickle shape.Sickle cell disease disturbs the delivery of oxygen.

Jaundice is a common symptom of sickle cell.Sickle cell does not live long like normal blood cells and they die faster than RBC.This make liver to filter them out.Bilirubin which causes yellow color from these broken down cells gets accumulated up in the body causing Jaundice.

Acute Hepatitis B does not require treatment and most adults clear infections very fast.Since this girls LFT is becoming normal it is recommended to continue her treatment as out patient.
