- Older adults/caregivers are often not aware of the great variety of services available to meet the health needs of older adults in their own home.
- The services must be culturally-relevant for people to use them.
- Important for nurses to address the lack of information about these in their pts.
- Address barriers such as negative attitudes of family caregivers

National Eldercare Locator- provides free info about many resources in the US based on zip code
Senior information and referral service- local services that provide info about agencies to address specific needs
Area agency on aging- gov agencies that provide a wide range of services (i.e. referrals, case management, and non-medical homeworkers)
Senior centers- community-based centers providing services for older adults (i.e. meals, limited transportation, and social/educational programs)
Home-delivered meals- provide home delivery of hot meals to homebound people, sponsored by local senior centers, churches or hospitals
companions and friendly visitors- offer services such as socially oriented home visits, assistance with errands, or accompaniment to appointments
Telephone reassurance- scheduled telephone calls to older persons to provide support and reminders
Personal emergency response system- emergency call button (i.e. life alert if a pt falls or has an accident)
Energy assistance programs- financial assistance for utility bills for people with low incomes
Home weatherization and home repair service- contractors paid by gov agencies to provide home repairs and maintenance for people with low incomes

Community based health promotion programs
-senior centers often offer periodic health screenings and health education activities.
-health promotion activities include BP checks, safe driving courses, smoking cessation classes, immunizations, medication assessment, management and education, various types of exercise.
-health education topics include nutrition, stress management, general health care, seasonal health issues (i.e. hypothermia), heat related illnesses (i.e. cold/flu).
-organized group activities (i.e. senior wellness programs).
