Medication :

1) Tablet Apixaban (Eliquis):

Apixaban belongs to anticoagulants, the drug that prevents clotting. Anticoagulants are classified based on their mode of action it includes : Vitamin k antagonists, Heparin, Direct thrombin inhibitors, Direct factor Xa inhibitors. Commonly prescribed dosage ranges from 2.5 to 5 mg per oral.

Indications of medications are : to treat stroke secondary to atrial fibrillation, to reduce postoperative complications of deep vein thrombosis and pulmonary embolism. The common side effects bleeding, anaphylactic reactions related to drug allergy, syncope.

Nursing implications : The most vital role of nurses is to educate that anticoagulants hinders with clotting mechanism, patient education focuses on prevention of injury, as even injury of minor forms will results in severe bleeding, to use soft bristle brush to prevent gum bleeding, to notify heath care provider incase of malena, hematemesis as it indicates internal bleeding. 2) Ferrous sulfate:

The side effects includes bloating, sleep disturbances, taste alteration, nausea. Nursing considerations includes: Encourage patient to take dietary supplements of food rich in folate.

4) Gabapentine :

Drug used to prevent and treat seizures and nerve pain . It is categorized under anti convulsants and anti epileptics , to control seizures. The first dose must be taken on bed time to overcome side effects. The serious side effects includes tremors, increased reflexes, slurring of speech, hyperthermia.
