It means,

4/6 = 2/X

Now cross multiply,

4X = 12

X= 12/4

X= 3

The value of X is 3.

Weight of the patient is = 168 lb = 76.2035 kgs.

~ 76.2 kgs.

Needed dose 15 mg/kg of body weight.

= 15 * 76.2 kg ( body weight)

= 1143 mg total drug is needed.

Desired dose= 1143 mg

Dose in hand = 250 mg.

quantity of dilution = 10 ml.

the volume of drug needed =

Desired dose / Dose in hand * quantity.

= 1143 mg / 250 mg * 10 ml.

= 45.72 ml.
