Health history collection refers to the collection of subjective date from the client that will help the health professional to make diagnosis and initiate the treatment promptly. The components included int he health history includes :

Demographic data : Name,age,sex,occupation,marital status.

Chief complaints- it involves in collecting data related to reason for the present admission. For ex) Chief complaints depends on the type of illness.

Past medical history: It includes past health history, medications,previous injuries,accidents,vaccinations,admissions.

Past surgical history: Surgery performed early.

Family history : Number of members in the family,any hereditary illness like diabetes,hypertension,cancer and congenital anomalies.

Nutritional history: Diet vegetarian/non vegetarian, height and weight to calculate BMI.

Allergy: Existing allergy towards any drug,dust and food.

Social History : smoking history, alcohol intake, marital status,occupation.

Review of systems : Cardiovascular system, respiratory system, gastrointestinal system,nervous system,genitourinary,reproductive, ENT.

Vital signs: Temperature,pulse,respiration and blood pressure.
