Limited data set allows those information which can be revealed to a third party without authorization of patient if certain conditions are met. Hence, these information must not give away the identity of the patient, hence they shouldn't be unique information. Email address, Social Security number,a phone number all give away the identity of the person whereas Date of Birth doesn't. So the answer Is D) Birth Date.
