Think about a time where you (or a family member) received disjointed care due to poor interprofessional communication. As a result of this disjointed care, write a job posting about what the perfect “care coordinator” would look like. What is your creative solution to increasing interprofessional communication? What will this person’s education be and what type of training will they need? Will they need to be well versed in billing codes and communicate with insurance companies? Will they need to be able to write prescriptions? Who would be their direct employer? Where would their salary derive from and how would you demonstrate that it is not only improving 'patient centered care' but also is a cost effective approach? What will be the official job title? What other job duties need to be considered? You may use humor if feel it’s appropriate.

The care coordinator plays a major role in patient care...

Communication between the health professionals is very important in order to achieve complete care for the patient by coordinating things rights from admission till the discharge. Each and every department must have a care coordinator. The care coordinator must have formulated checklist in order to ensure that the things are going in a right way .... the checklist must consists of details coordinated will departments and staffs who are associated with patient.

The education required is that mostly the hospital management... as he is not the personal care taker of the patient he does have to know all the nursing care, etc . But he needs to know regarding all the departments of the hospital in order to coordinate with the departments... communication training and orientation with the departments of the hospital must be done..

They must be well versed In billing codes and also should communicate with the insurance companies in order to help clients in financial aspect... not all the patients will be economically sufficient for paying the bills of the treatment. Especially for the patients who are admitted for a long time. Care coordinator can help the patient and family with insurance eligibility and coordinate with insurance department.

They dont have to write prescriptions... care coordinator job is to coordinate with all the department to ensure complete care for the patient.
