One of current challenge in our health care organization in community :

There are so many challenges like

* financial challenges and hospital productivity : due to financial problems all the people may not get adequate health care.

* patient safety : in some hospitals due to financial burdens, pressures in the hospitals or organizations, some times abscand may taken places in the organizations. So that patient safety can not able to maintained in some hospitals.

* hospital security : due to any mistakes happens towards the clients, the relatives may quarrels to wards to the organizations.

* patient satisfaction : even though good care provided by organizations some times the patients may not get satisfaction.

* rules and regulations and standards of hospitals.

Apart from these the main problem is transportation facilities during emergency situation.

* due to lack of transport facilities from organizations to the community aears the clients may not reach immediately when they need to go to hospitals.

* and they can not get immediate care, which may effect on the population health.

Role of nurse manager in health promotion and disease management :

* nurse manager can play duel role those are supporting the nursing staff and administrative work.

* to promote health she will provides preventive care like vaccination and health education regarding preventive measurements like healthy lifestyle, eating habits and behaviors etc.

* she can coordinate the care among all health care workers to provide good quality of care to the clients.

* protect the people by giving education like environmental hazards , and mobilizing them to emergency situations.

* she can lead the health care organizations by using evidenced based practices.

* promote the equity of health to all individuals and communities.

* she can provide the integrity of work and care like physical, emotional, intellectual and spiritual.

* she can help and guide the health care staff to provide quality of care to clients.

This is the role of nurse manager in health promotion and disease management of patients.

The leading indicators of the population includes physical activity, tobacco use, alcoholism, mental health, immunization, substance abuse, over weight and obesity, injury and violence, access to health care, environmental quality.

These are Main leading problems of families and communities, even though by giving health education and also preventive measures due to some of the cultural and genetics history and negligence the people are suffering from diseases.

* and due to lack of knowledge and technologies and financial problems they are not receiving or accessing adequate care.
