Incidence report refers to an uneventful event happens to a patient admitted in a hospital,nursing home. The unusual event occurs results an injury to the patient.

Examples:

1) Delay in receiving medications: A diabetic patient due to receive insulin by 8 am but due to nurses negligence the dose of insulin is missed. This contributes to poor glycemic control and also increases HbA1c. It also increases the risk of complications.

2) Failure to apply bed rails for an elderly patient with altered sensorium results in fall injury.

Incident report on fall injury as a result of nurse negligence in applying bed rails for a patient with altered sensorium:

- Patient information : Name,Sex,Age,Location of incident.

- Incident information : Incident date , Day of week, Time of incident, Describe the nature of incident, Witness of the incident, Corrective action done.

- Bodily injury information : cause of injury, types of injury.
