Nursing assesfocuses on gathering data about a patient's state of wellness, functional ability, physical status and response to actual and potential health problems.

Data collection is a process of compiling information about the client.In this both subjective and objective data is collected.

Subjective Data : They are Symptoms or hidden cues which are the client's feelings and statements about his/her health problems.

Example:

i) Pain : Client reported that i have pain in abdomen.

ii) Anxiety: Client says that I dont know how much time it will take to recover my disease.

iii)Chief complaints : I am having difficulty in breathing.

iv) Functional limitations: I cannot meet my basic needs because of fracture in leg.

Objective Data: These are signs or open cues which are observable, detectable and measurable.

Example:

i) Physical examination: observation, palpation, percussion and auscultation

ii)Laboratory investigations: blood reports, urine and culture report, gene expert report etc.

iii)Imaging and Diagnostic Studies: MRI, CT Scan, USG

iv) Body measurements: Height, weight, Chest, Arm and Head circumference etc.
