  • The clinical documentation improvement  training on ICD 10 and PCS has to be implemented
  • The importance of coding should be enforced to the physician
  • The queries should be related with the clinical practice
  • The physician on internship, practitioner, mid level provider has to be provided knowledge as per the standard
  • It has to be included in physician workflow daily
  • In person communication with the physician can solve the issues easily
  • Immediate and proper feedback to be given.
  • Give an evidenced base reasoning and reporting
  • Performance monitoring
  • Educate through documentation training session.
