10. Based on the CKD - API equation CKD is Stage 4. Patient is experiencing GFR less than 60 mL per min should be more tjan 3 months. Calculated GFR is 21 mL per Min. Patient to be refered to Nephrologist. Risk of fast progression is very high. 3 months once consultation to be done. UACR ratio of 500 mg/ g suggests that albuminuria is present.

11. Based on the CKD- API equation CKD is Stage 3. Patient is experiencing reduced GFR of 37 ml per min for more than 3 months. Risk of progreassion is moderate. Referral to nephrologist is optional. 6 months once consultation to be done. UACR 289 indicates microalubminuria.
