1. The care process plays important role in a healthcare setting. The every step in treatmeny plan is crucial. After prescribing the medicines the pharmacist has to make sure that all the prescribed medicines are delivered to the patient or not. And he should say the details of use, frequency, ROA and side effects of medication. If he is an inpatientthen the nurse in the particular unit should note down the frequencytimings and make sure that he has taken the tablets. If he is an outpatient the nurse or any other members of healthcare team should call them regularly and has to note down his condition. And they should also know that he is daily his medication or not. In special conditions or for deadly diseases the patient should be reffred to a nutrionist and give them complete details about what to take and what not take take while taking the medication or having the disease/ disorder.

The data entered in the suitable softwares using a computer is very usefull for quick analysis.the variations occured may in the following ways- social, Hearing, me,or virtual.

2. After interviewing the healthcare professional it came to an end that direct patient counselling and awareness programmes worked well in the community other than telecommunications and all.
