1. What does Forms Design and Screen Design have to do with Standards?

2. An EHR is filled with a wealth of information that is just waiting to be extracted and reported on. Think of a topic that you could run a report on from a set of health records.

3. Explain why a clinic would want to know this information (what benefits would the clinic have in gathering this information and running these reports).

An example: Topic: the number of teenagers who smoke. A clinic could see if this number is increasing or decreasing. If it's increasing, it might want to have the clinical staff be more diligent in their smoking cessation counseling services. If it's decreasing, it might use this information to advertise that the number of their teenage patients who smoke has decreased (thus inspiring other teens to not start, because it's 'not cool')

#. Steps to be followed by making a form - Determine the types of information that must be captured in the fields of the form (the form's purpose). Arrange those information types in a logical sequence and ensure that all information types are there and not repeated . Apply that arrangement to the creation of a form template. Customize each field on your form template. Test drive the form.

#. Basic design principles regarding selection and sequencing of items (paper forms and screen views)

Effective and efficient sequencing of the form or view should follow the traditional pattern- from left to right and top to bottom- permitting a continuous operation whether reading, writing, or entering data on the computer. The flow should be logical and take into consideration the order of data collection or transfer. In addition, numbering items makes references to both items and written instructions on completion of forms faster and easier.

Terminology should be standard and understood by all, abbreviations should be understood by all, and master formats should be used to promote consistency.

Forms and views should be kept simple, so they are not overwhelming to users. They should have a specific purpose and be created only when necessary.

Headers and footers should include important company information (phone numbers, scanning codes) as well as information about the form (page numbers). Dates of forms should be included, as well as patient names and record numbers.
