Answer :

Nursing interventions of an abnormal results of the following :

1. Complete blood count.

* It includes all red blood cells, white blood cells, plate let's, and hemoglobin levels ect.

* If less hemoglobin levels means as nurse we need to educate the client regarding intake of Iron rich diet to correct anemia.

* Like all green leafy vegetables, egg, milk, drum sticks, all black fruits may increases the urine.

* if there is any increased wbc count indicates infection, so nurse has to educate regarding maintainance of aseptic techniques .

* educate to take vitamin c diet absorption of iron.

* if it is severe anemia blood transfusion may done.

* if there is any Thrombocytopenia advised to take papaya and pineapple juice to improve platelets.

* advice to use soft tooth brush to prevent any bleeding from gums.

* plate let count should be done twice a day up to improvement more than 1.5 lakh.

2. Hgb(hemoglobin) and hct(hematocrit) :

If these leaves are low there is a anemia. To find out anemia we can check this hgb and hct levels.

* adversed to take iron rich diet.

* provide adequate diet and rest to prevent fatigue and weakness.

* administer oxygen to maintain normal breathing pattern, because low hemoglobin leads to less oxygen carrying capacity leads to dyspnea.

* provide Fowler position.

3. Coagulation test :

* if there is increased values of anticoagulants like PT, the bleeding disorders may increases.

* don't administrator aspirin and warfarine and heparin drugs to this type of clients.

* don't give intramuscular injections.

4. Creatinine :

If these values increases, indicates renal problems.

* administer diuretics.

* avoid to give high fluids due to lack of elimination.

* provide adequate rest to avoid weakness and fatigue.

* administer anti emitics like ondonsetron, to reduce nausea and vomiting.

5. Serum electrolytes :

Includes sodium, potasium, chloride, magnesium calcium ect.

* administer electrolytes through intravenous fluids.

* investigate continueously for levels of those electrolytes alterations.

* administer multivitamin injection, it may decreases some neurological problems due to electrolyte imbalance.

* provide comfortable bed rest.

* advise patient to take adequate oral fluids like juices ect.
