It is absolute medication error happened in the surgical room..The syringe that was supposed to contain lidocaine instead has been filled with topical adrenaline,a medication that is never supposed to be injected..

Ben's death made changes in procedure at martin memorial medical center..Drugs no longer poured from a bottle into a cup and then transferred to a syringe,a special filtering device is now used to transferred medication directly to a syringe..The transfer should be under the observation of more than two nurses,they must verify the contents and no vials to be disposed until the surgery finished and patient is stable..

Yes, Ben's death made lot of changes in this hospital..To reduces the medical error in surgical room they stated using pulse oximeter,which measure oxygen in blood..it helps Physician to take steps to prevent medical errors..

As a patient I can appreciate the way it was handed..but they can not get back Ben again..as a patient we believe only Physician as a god for our illness..standards and protocols will make the hospital strict with follow up.. hospital should follow their culture while handling patient and they should eradicate medical errors,malpractice completely to save one's life..
