1) Patient safety plays a significant role in the hospital and it must become the priority always as the comprehensive approach of care is always patient centered. The elements include :

# the drug administration responsibility : when the responsibility and the accountability is being given to the staff the safety will be ensured

# Patient Guided direction (PGD) drug delegation: the pgd drugs cannot be delegated to anyone other than the professionals who are responsible this also in turn reduces the harm and improves safety

# prescribing, transcribing and supply of the drugs: the prescription only medications to be followed in hospitals if to be transcribed than the registrant must have signed and if transposed the pharmacist has to be involved.

# establish a blameless environment : a open culture, transparency, constructive feedback for the better patient safety to be fostered.

# raising the safeguarding concern when a patient is facing the adverse effects

# identifying the potential harm involved in the drug administration and preventing it.

This contributes to a non punitive approach of having the more instances of adverse effects including the fatal anaphylaxis, drug interactions and it prevents patient from long term drug related disabilities. The administration buy in is very important to have the goals achieved in providing the treatment to the patient and have a better health. It must be supported by the administration for it to be effective as they are the one who are capable and able to plan the budget and take into consideration and face any consequences as a team.

2) Ben Kolb died because of an error in the medication administration which lead him to death.

The lessons learned by Martin memorial are as follows:

# they have the center for patient safety that would reduce the error reduction.

# children entering the emergency presenting their signs and symptoms who require defibrillation, drugs are monitored for the body weight. And are given a wrist band of a color that marks the medicines and the equipment settings.

As a patient I would appreciate atleast the seriousness of the medical errors would be controlled and reduced with these strategies and helps the patients have risk free and appropriate treatment required for them according to their condition.
