You have been hired as the new Risk Manager at WeCare General Hospital. You were hired mainly due to your experience in creating a High Reliability Organization. As CEO of WeCare General, I have received an executive brief on the top 10 patient safety concerns in healthcare. The number 1 concern is Diagnostic Errors. You are being asked to write a recommendation for me outlining the steps you will take to determine if this may be an issue at WeCare General. How do we monitor this going forward? Support your plan with resources

The recommendation should be 2 pages long with bibliography on the 3rd page. Include article searches you have done so far as part of your bibliography.

  • The diagnostic problems occur due to certain reasons
  • Due to lack of coordination among health care team
  • Diagnosis should be done in a proper channel
  • Lack of communications between health care team and patient - family
  • Lack of evaluation after diagnosis a patient and less disclosure after diagnostic errors
  • How to overcome this?
  • Proper education to patient and family memebers. They should be aware of the how they did the diagnosis, signs, symptoms, and future care
  • Always explain the procedure towards the patient. The client has the right to know all procedure
  • Proper training for doctors, how to find the disease of a patient. The doctors should be aware of the advanced technology and treatment options. They should have thorough knowledge to clarify the doubts of the patient
  • Continue Medical Education to update the knowledge is recommended
  • They should get proper feedback regarding diagnosis they made and suggestions for improvements, if necessary
  • Proper training to handle advanced technology. They should be well trained to handle IT. They should know if any errors occur due to IT . They should have options to pick up the technology. It should be convenient and reliable
  • Coordination between IT department and Vendors is highly recommended
  • If any technical errors occur, clinicians should be aware, how to inform the concerned authorities
  • Quality assurance department is necessary in a hospital with review meeting twice or thrice in a month
  • Implement protocols how to follow if any diagnostic errors occur and evaluate the success rates
  • Target areas that is higly required for improvement
  • Design a work system , such a way that , health team members will follow to prevent diagnostic errors
  • Approach a non judgemental policy if any diagnostic errors occur
  • Health care team members should be aware of legal aspects during patient care and conducting diagnosis
  • Planning is necessary to prevent diagnostic errors
  • If any clinician is repeatedly conducting diagnostic errors, encourage him for training. Unless his training is completed with fruityful results, he should not be allowed to diagnose the patient. As client is the important person in every aspect. However, do not consider this as a disciplinary actions because mistakes will happen and nobody is perfect in this world.
