Please don't copy and past need appropriate answers need ASAP

As a Risk Manager hospital is facing a Diagnostic Errors problem. You are being asked to write a recommendation for me outlining the steps you will take to determine if this may be an issue at WeCare General. How do we monitor this going forward?

When a diagnosis is accurate and in a timely manner , a patient has the best opportunity to positive health outcome because clinical decision making will be tailored to a correct understanding of the patients health problems.

Diagnostic error is a biggest mistake and will have negative effects on both the patient and healthcare care provider. It needs to be resolved quickly as it raises questions towards the ethics too.

As a risk manager ,I will first of all find the source of error means from where it's emerging is it from the outside labs ,or inside labs of hospital itself ,whether it's the mistake made by staffs in confirming the diagnosis or was it written wrong etc .

Then we need to develop and follow a diagnostic process to make a correct diagnosis . The diagnostic process step by step includes :-

1.- First patient experiences health problems

2.- Patient engages with healthcare system

3.- Healthcare providers gather information includes:-

a) clinical history

b) physical examination

c) consultation and referrals

4.- Information integration and interpretation

5. Diagnostic testing and collection of reports in written by the doctor.

6. Communication of diagnosis oon the basis of reports to the patient.

After developing the diagnostic process ,it should be included in the policy of the hospital that no diagnosis will be made without following these steps and it will be considered as punshible offense if does not followed by the staffs . Proper training of staffs has to be given and a demo must be organised for the staffs on the positive aspects of following the process and the negative outcomes due to not following the steps .

Regular monitoring of staff regarding correct diagnosis making ,are they writting the correct diagnosis in the patient's records ,no overwritting allowed ,double checking of diagnosis by staff because every disease has different treatment protocol and incorrect prescription will cost the life of patient.

To make them follow the diagnostic process first of the staffs must be explained about the importance ofaking correct diagnosis and the consequences patients have to face due to incorrect diagnosis . Staffs has to place themselves in the palace of patient and evaluate the circumstance. It will motivate them to use the process and will result in positive health outcome.
