Work from Patient Safety course please don't copy and paste ?

1. Since 2013 how many states now require reporting of adverse events. Does Missouri? If you live in another state, does your state? Why would a state choose to not implement mandatory adverse reporting? As a patient, how do you feel about this? What about as a potential healthcare practitioner or administrator?

2. Discuss the reasons that low health literacy impacts patient safety. What do you think can be done to improve this? Is improving literacy the responsibility of the healthcare organization? Why? If not, whose responsibility is it?

3. The author listed four steps for providers to keep in mind when using HIT implementation as a tool to improve overall patient safety efforts. Briefly explain the purpose of HIT implementation. Who is responsible for HIT implementation? Why? How much input does the normal practitioner have in this?

1.To err is human. Adverse events are any undesirable act while giving healthcare leading to any injury /illness may be due to negligence or due to unavoidable effectsExample:Near misses during medication administration , Any healthcare provider or patient injured as a result of a medical device failure or its misuse.

About 27 states has implemented the adverse events reporting system.Minnesota was found to implement the adverse event reporting system as mandatory due to 28 serious reportable events which occured previously.Missouri doesnot have adverse event reporting system . Missouri has a commission on Patient Safety which marks medical malpractice, malpractice insurance, and the balance between public accountability for the quality of health care and the need for health care providers to learn from medical errors.The commision recommended a private Patient Safety Center for Missouri. Missouri hospital association participates in statewide and national initiatives to improve care and safety .

Adverse events happen because of a group of events ,policy and people. Not a single doctor or healthcare provider may be pointed out . Whole hospital should be a team to provide quality care and deliver patient safety. Health promotion can be achieved by patient safety and access to good care.
