Objective Data

  • Decreased blood pressure(Hypotension)
  • Inactive
  • Decreased skin tonicity
  • Tachycardia

Nursing Intervention

  • Check hemoglobin

Monitoring hemoglobin helps to identify if the patient is in need of blood transfusion.

  • Check blood pressure and heart rate

It helps to identify the patient's volume status as hypotension and tachycardia occur when the blood volume is low.

  • Provide blood transfusion if required.

Hemoglobin level below 8 g / dL requires a blood transfusion.

  • Monitor ECG levels

A low volume of blood interferes with the functioning of the heart.

  • Monitor for bleeding in the stools

Black/tarry stools indicate upper GI bleed and bright red blood indicate lower GI bleed.

  • Prevent fall or injury for the patient.

Due to hypovolemia, there is a chance of falls. Assisting them in their activities should be done.

Expected Outcome

To control the bleeding and to normal the vital signs.
