1) ICD-10-CM, coding conventions dictate that for coding an inpatient hospital stay, the principal diagnosis, the diagnosis arrived after study is the code that is listed first. In the outpatient setting this is not the case. In the outpatient setting, the study of the patient's disease may make many visits and have many tests and other procedures before the diagnosis is determined. In the outpatient setting instead of a principal diagnosis, it is instead the first-listed diagnosis. What are the outpatient coder's guidelines for selecting codes?

1. The guidelines to be followed in outpatient services are:
• Selection of first listed condition like outpatient surgery or observation stay because in outpatient services diagnosis is not established at the initial visit. It may take two or more visits.
• Codes from 001.0 to V84.8 is used
• The diagnosis should be reported accurately.
• Codes 001.0 to 999.9 is used commonly
• Codes are used for signs and symptoms
• Codes are given to circumstances other than the disease or injury
• Do not code uncertain diagnosis
• Chronic cases are coded for several times
• All the documented conditions are coded at the outpatient visit
2. 76700 is the first cpt code in urinary system. It is given for ultrasound of the urinary system. Code 52310 is used for urethra and bladder transurethral surgical procedures and for simple cystourethroscopy and code 52315 is used for complicated cystourethroscopy. Lithotripsy is a procedure that uses shock to break the kidney stones. After this procedure the tiny pieces of stones are passed out of the body through urine.
3. The most common endocrine disorder is diabetes. Acromegaly is a hormonal disorder in which the pituitary gland produces too much of growth hormones. The features are enlarged hands and feet, coarsened, enlarged facial features, coarse, oily, thickened skin, excessive sweating, increased body odur, skin tags, facial and muscle weakness, husky voice, snoring, impaired vision, enlarged tongue and barrel chest. The life threatening complications are cardiomyopathy, spinal cord compression and hypertension.
