You are an RN at West Coast Hospital, on the pediatric unit, 2 North. Today, you are caring for patient Minnie Mouse, Medical Record number: 0795236 (DOB 1/12/2010), her Attending MD is Dr. Smith. Parents, Donald Duck and Daisy Duck, have been by her side this admission. The following events occur during your shift.

  1. A 20 Gauge angiocath was placed at 1330 in the right antecubital (AC).
  2. Normal Saline (0.9% NS) Bolus of 500mL was started. At 1335 client IV site was reassessed with no s/s of infiltration, redness, swelling or pain to right AC.
  3. At 1402 nurse evaluated IV site and moderate swelling was present, arm was cool to touch and 200 cc had infused. Patient was complaining of 4/10 pain in arm. CSM to distal extremity was intact.
  4. IV fluids stopped immediately, IV discontinued, angiocath intact.

What went wrong with the IV, I NEED TO FILE INCIDENT REPORT.

The patient had infiltration due to the infusion (swelling, cool arm, pain are indicator of vein infiltration)

The incident report should describe the patient status due to the infiltration occired because of intravenous fluid transfusion.This can be because dislodged catheter, excess movement of the arm ,injured vein.
