CARE COORDINATION : Care coordination is defined as the activities which are organized by a team of personnel members to deliver the appropriate services to patient, information regarding health care settings. It connects both services and resources. Care coordination connects inpatient and outpatient care by maintaining of all the records and reports of patient on admission into the hospital so that that information can be used after the discharge of patient for re visiting or follow up to the hospital as a outpatient which reduces the cost of care and conservation of patients medical records will be easier for the future needs.
