6. One must be cautious with administering oxygen to Mr. C.Y.because

  • the pneumothorax or collapse lung could lead to tear the lining again.
  • Presence of emphysema limits the air flow across the lungs.
  • Stiffened alveolar sacs cause the mininimal processing of amount of air that enters the lung and continuous monitoring is required.

​7. The three complications from immobility and their solution are :-

  • DVT - deep vein thrombosis in legs may develop from immobility due to inadequate circulation of blood. This can be prevented through ankle toe movements, pressure stockings and getting up and out of bed once or twice in a day.
  • Increased respiratory secretions and their accumulations - immobility leads to increased production of respiratory secretions that gets accumulated due to insufficient elimination of these secretions. This can lead to respiratory infections. Such problems can be dealt with postural drainage, breathing exercises, teaching of huffing and coughing techniques.
  • Generalized muscle weakness - immobility leads to generalized muscle weakness due its disuse. This can be prevented through active exercises of upper and lower limb and encouraging patient to have short walks or use toilet whenever possible.

8. Emphysema or COPD may sometimes lead to congestive heart failure. Emphysema leads to low oxygen saturation levels in blood. The heart has to work more to meet the oxygen demands of body tissues and organs. This overloading of heart leads to congestive heart failure.

- breathing exercises like pursed lip breathing

- huffing and coughing techniques

- respiratory muscle training

10. Mr. CY may not get proper nutrition because :

- he is elderly and may not be able to cook for himself

- he may smoke more which is bad for his health

11. Other support measures include enrolling Mr CY in recreational activity groups to boost his psychological status, physical therapy to improve cardio respiratory endurance, educating his family and caregivers and ensuring that he is being taken care properly, dietary modifications.
