When a nurse in duty is attending all the patient should assess them on the basis of specific problem and know its progress or detoriation.A comprehensive health assessment is not needed everytime of there is a long stay patient whose entire history os known to the nurses,hence a nurse for a quality and a best outcome care should conduct a focused assessment on each patient.
