Age - 25 years / M

House address - 45 behind luke valley builders , 24 street campus , New Jersey

Education - 12 the pass

Occupation - works in a cotton mill

# Chief complaints :-

- persistent cough for 1 week

- breathing difficulty

# History of present illness :-

The patient was perfectly all right one week back . Then suddenly one morning he got started having cough , extreme cough that tears came out of his eyes and he had shortness of breath . He tired many home remedies o get rid of cough but was not effective and reached the hospital

# History of past illness :-

No habit of smoking and alcohol drinking

No history of hereditary disease

# Physical examination :-

Include a thorough head to toe examination of organ . After having the physical examination done the patient was referred to respiratory department because on auscultation he was having wheez and crackles like sound along with breathing difficulty and cough which was non productive.
