Understanding family structure and style is essential to patient and family care. Conducting a family interview and needs assessment gathers information to identify strengths, as well as potential barriers to health. This information ultimately helps develop family-centered strategies for support and guidance.

This family health assessment is a two-part assignment. The information you gather in this initial assignment will be utilized for the second assignment in Topic 3.

Develop an interview questionnaire to be used in a family-focused functional assessment. The questionnaire must include three open-ended, family-focused questions to assess functional health patterns for each of the following:

1. Values/Health Perception
2. Nutrition
3. Sleep/Rest
4. Elimination
5. Activity/Exercise
6. Cognitive
7. Sensory-Perception
8. Self-Perception
9. Role Relationship
10. Sexuality
11. Coping

Select a family, other than your own, and seek permission from the family to conduct an interview. Utilize the interview questions complied in your interview questionnaire to conduct a family-focused functional assessment. Document the responses as you conduct the interview.

Upon completion of the interview, write a 750-1,000-word paper. Analyze your assessment findings. Submit your questionnaire as an appendix with your assignment.

Include the following in your paper:

1. Describe the family structure. Include individuals and any relevant attributes defining the family composition, race/ethnicity, social class, spirituality, and environment.
2. Summarize the overall health behaviors of the family. Describe the current health of the family.
3. Based on your findings, describe at least two of the functional health pattern strengths noted in the findings. Discuss three areas in which health problems or barriers to health were identified.
4. Describe how family systems theory can be applied to solicit changes in family members that, in turn, initiate positive changes to the overall family functions over time.

Cite at least three peer-reviewed or scholarly sources to complete this assignment. Sources should be published within the last 5 years and appropriate for the assignment criteria.

Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

Family health assessment involves a set of actions by which the status of a family as a client, its ability to maintain itself as a system and functioning unit, and its ability to maintain wellness, prevent, control, or resolve problems in order to achieve health and well-being among its members are measured.

An assessment is a process for gathering and organizing information. A family assessment does this in ways that can help a family prevent and solve problems. It aims for a full understanding and unbiased view of the family—not just its problems, but also its strengths, values, and goals.

Values/Health Perception	Used to practice health promotion activities such as regular checkups and screening, and engaged in health promotion activities
Nutrition	Use to eat most of the food items, I like to eat fish and mostly I used to avoid highly saturated oils
Sleep/rest	Used to sleep a minimum of 6-8 hours at night, and rest for 2 hours after lunch
Elimination	Normal elimination pattern(2 days), voiding is the normal minimum of 1500-1800ml
Activity/Excercise	Exercise daily 15 minutes morning and evening, Participate in ADL's without any assistance
Cognitive	Memory is normal and actively participate in the decision-making process
Sensory perception	Vision, hearing, taste, touch, smell, pain perception are normal
Self-perception	Perceiving as normal such as body image, attitude about self
Role/relationship	Role as wife and mother and maintaining a good social relationship
Sexuality	Satisfaction with a sexual relationship and have a history of 2 pregnancy with 1 childbirth and 1 abortion
Coping	Handling stress in a positive manner and control and manage all the situations whatever faced

Mrs.K family is a nuclear family, living with her husband and a child boy. She is an African American and her family belongs to a middle class. She believes in God and is giving importance to her environment and surroundings and maintain social-relationship with all the members of the community whoever she knows. The overall health behavior of the family seems to be good. They used to practice healthy eating pattern and lifestyles and used to participate in health promotion programmes and usually did the annual check-ups and screening for diseases.

Beliefs about the nature of health and factors affecting it or the causation of a current illness can provide insights into beliefs about disease causation and how the disease should be treated. There is no history of use of alcohol, cigarettes, and drugs of abuse. The person's usual dietary pattern and timing of meals should be scheduled, Patterns of excretory function are normal. The amount type of physical activity involved in the patient daily life - - either at work, as part of exercise routine, seems to be good. Assessment of a patient's usual patterns of exercise can provide clues to overall health-related lifestyle as well as detecting any extremes.

#The importance of family system theory is to help; within the boundaries of the system, patterns develop as certain family member's behavior is caused by and causes other family member's behaviors in predictable ways. Maintaining the same pattern of behaviors within a system may lead to a balance in the family system, but also to dysfunction.

A nuclear family tends to experience issues in four main areas such as :

-intimate partner conflict

-problematic behaviors or concerns in one partner

-emotional distance

- impaired functionality in children.

Anxiety may lead to fights, arguments, criticism, under- or over-performance of responsibilities, and/or distancing behavior. Though a person’s particular belief system and attitude toward relationships may impact the development of issues according to relationship patterns, Bowen held them to be primarily a result of the family emotional system.
