1.SDOH is social determinants of health . The major five social determinants are :-
Biology and genetics. Examples: sex and age.
Individual behavior. Examples: alcohol use, injection drug use (needles), unprotected sex, and smoking.
Social environment. Examples: discrimination, income, and gender
Physical environment. Examples: where a person lives in crowding conditions
Health services. Examples: Access to quality health care and having or not having health insurance .
Each factor of social determinant is important and will affect the family health status because health is composed of all these social components . If any factor is disturbed then it will directly affect the health of the individual and when the individual is affected ,the family is also affected and hence the society and community because all these are connected with each other .
For example - if an family member has an habit of drinking ,it will affect his life and also the other family members life who are attached to him .
Screening of the family members should be according to the age group because the requirement of each age groups people is different and also their health status . So, when we do the screening for health status it should according to the specific age group.

I am choosing the public health model of prevention which describes the methods to prevent , promote and restore health . It consists of the four major components :-
Primordial prevention example healthy lifestyle
Primary prevention means actions done to prevent diseases like immunization etc
Secondary prevention means early diagnosis and treatment of disease
Tertiary prevention means providing rehabilitative care to decrease disability and discomfort .
Following this health model is beneficial for the family because it will help the family members to follow a good healthy lifestyle and do the modifications necessary for having a disease free life and also this models helps the individuas to how to early diagnose and get treated for the disease and use the rehabilitative care to improve their health status .
