To Implementing quality program the best practices to get attention..Educate all clinical staffs,and provide falls prevention training for clinical staffs and rehabilitation services.. explain about Patient risk factors for falls.. improve handoff tools between department and each shifts..utilize staff skills and strengths into allocating responsibility..The fall reduction program includes Patient evaluation,intervention,staff education and training,patient education and outcomes assessment.

Have a set of goals to identify high-risk medication and uses system,implement changes that reduce the number of medication error which is harmful..communicate regularily with staffs and increase awareness of medication error safety metric.strategic plan should be approved by pharmacy and therapeutic committee to increase awareness,feedback,buy-in and support from multiple disciplines..

Major challenges face in health care system is health professionals have to be prepared.implementing health care and all medical professionals to take part in program..lack of stakeholders and government participation to provide financial needs making the program down..
