The senario:

On my first day, the nursing manager orientate us to the unit 4D, patients mostly stroke clients, some are amputated, it was a 7am to 3pm shift.

On second day, in the same unit with a different instructor, did a head to toe assessment on a 62 years stroke patient, make her bed and transfer her to the chair as she requested.

On third day we went to the same unit, another different instructor, did the morning vital signs, blood glucose, and showed as some instruments like decompression machine, foley, NG tube.

On the last day which is the fourth cycle, we went to Med surg floor at unit 5D, I was attached to a nurse preceptor with 5 patients, i gave medications, and did documentation.

We were posted for our practical experience in the rehabilitation center at Jacobi Medical Center in New York. It was a 4 day posting . We were posted in the 4D unit for first three days and last day in the surgical ward 5D unit

Majority of the patients in 4D unit were elderly and were admitted as a result of stroke . First the nursing manger gave us the orientation about the ward and different disease condition which we can see their. The timings of the shift was 7am to 3pm . Next day we were again posted in 4D ,and I did physical examination of a 62 year patient and helped and made her bed and transferred her bed to chair on request .

Third day i was again posted in 4D , that day I tool the vital signs , blood glucose , and our nursing instructor thought us about some instruments like decompression machine, Foley's tube etc. Fourth day , our posting was changed to surgery ward 5D , I was working with a senior nurse preceptor and looked after 5 patients ,gave their medications including oral and intravenous and did the charting in the patients file .
