Injury and Illness Incident Report :

Betty a Registered Nurse during her shift , found her self uncomfortable with the bags of old documents placed in the Nursing station. Each time when she has to enter the data in to EMR she has to cross all the things which made her too difficult to complete her work.So she decided to place all the bags on a small roofed portion in the Nursing station.So she placed a ladder to climb on it so that she can keep them in place.She took support from other nursing assistants.

She is on the ladder, taking the bag each one by one and placing it on the roof. Mean while when she is taking the last bag , the bag slipped off .In an effort to avoid the files dropping out on the floor she tried to hold the bag forgetting that she is on the ladder.She missed balance as the ladder slipped off on the floor slightly.Betty fell down all of a sudden from nearly 10 feet.As the ladder slipped she totally fell down along with the ladder.

So had blunt injuries which are ladder associated and also some parts of the body like elbow, knee were hardly strike to the ladder and also to the floor.She had more pain in the ribs, soreness of elbow and knee, causing more pain on touching the part.She could able to move her wrist as it's strained in the injury.
