Case study

Mr. Finch is a single, 27-year-old, paraplegic veteran. He was injured 8 months earlier while riding in a jeep that hit a roadside bomb. On returning home from his military service, he began rehabilitation and has been learning to adapt to life confined to a wheelchair. Mr. Finch uses a straight catheter to empty his urinary bladder three or four times each day. He was admitted to the hospital yesterday for treatment of a persistent urinary tract infection. When asked, Mr. Finch states that he “prefers to be called Tom.” He answers questions abruptly with one-word responses and expresses a lack of trust in his physicians and nurses.

Tom has a high school sweetheart to whom he is engaged, and she visits him each evening after work. During the day, Tom makes an occasional sexually suggestive remark to his 24-year-old, female registered nurse. Tom asks the nurse for her personal contact information and permission to call her for help with his care after he is discharged from the hospital.

Question

1. Name four ethical concepts that, when implemented by the RN, will help Mr. Finch establish trust in his health care providers.

2. Identify a charge that could be made against the registered nurse if she engages in a social relationship with Mr. Finch that violates nursing standards of care.

3. Write four interventions that the registered nurse should initiate to avoid crossing professional boundaries with Mr. Finch.
