1) Patient weight = 44 lbs

Ordered dose = 325 mg stat

Recommended dose of Tylenol = 10 - 15 mg/ kg/ dose or 10-15 mg/2.2lbs/ dose ( 1 kg= 2.2 lbs)

Available dose = 160 mg / teaspoon or 160 mg/ 5 ml ( 1 teaspoon= 5 ml)

A)Calculation of safe dose:-

By proportion method :-

10mg : 2.2 lbs :: X : 44 lbs ( using lower limit of recommended dose)

X = (10mg × 44 lbs) / 2.2lbs = 200 mg

15 mg: 2.2 lbs:: X : 44 lbs ( using upper limit of recommended dose)

X =( 15mg × 44 lbs)/ 2.2 lbs = 300 mg

Safe range is 200- 300 mg/ kg or 2.2 lbs/ dose

Yes, this order dose is safe.

B) By proportion method:-

160mg : 5 ml :325 mg : X ml

X = (325 mg× 5ml ) / 160 mg = 10.15 ml

Hence, 10.15 ml would be given.

2) Ordered dose = 185 mg

Available dose= 100 mg / tsp or 100 mg/ 5 ml ( 1tsp= 5 ml)

By proportion method:-

100 mg: 5 ml :: 185 mg : X ml

X = ( 185 mg × 5 ml)/ 100 mg

X = 9.25 ml

Hence, 9.25 ml will be needed.

3) Client weight = 110 lbs

Ordered dose = 2.5 mg/ kg or 2.5 mg/ 2.2 lbs

X = ( 2.5 mg × 110 lbs)/ 2.2 lbs

4) Ordered dose = 15 mEq of Kcl

Available dose= 5 mEq/ 10 ml or 5 mEq/ 2 teaspoons ( 5 ml= 1 tsp)

By proportion method:-

5 mEq: 2 tsp::15 mEq: X teaspoons

X = (15mEq × 2 teaspoons)/ 5 mEq

X = 6 teaspoons

5) Ordered dose = 4 mg IM

Available dose in powder form = 50 mg

After reconstruction , available dose= 50mg/ 5 ml

By proportion method:-

50mg : 5 ml:: 4 mg : X ml

X = ( 4 mg X 5 ml)/ 50 mg

X = 0.4 ml
