Q1) a)  Acute pain: this is due to sickling of RBCs and aggregation in capillaries, preventing adequate flow of blood and oxygenation, as evidenced by patient reporting/ grading the pain and reduction in physical activity.

b) Altered cardiovascular tissue perfusion - due to the peripheral pooling of blood in the spleen as evidenced by tachycardia, increased respiratory rate and mildly decreased blood pressure.

c)  Fatigue - due to sickle cell anemia as evidenced by patient's lethargic physical movements and need to sleep.

Q2) Yes, we have to include her concerns on healthcare availability based on financial status - educate her about the insurance rights - medicare or medicaid whichever applicable based on her financial status.

Q3) Involve - 1. Opthalmology dept - to check retina

2. Surgical / medical gastroenterology dept - to assess splenic health

3. Neurological dept - to assess CNS function

4. Cardio - pulmonary dept.
