Wound
Wound is an injury such as a cut or stab or tear to a part of the body.
Types of wounds: There are different type of wounds like stab/ puncture wound, surgical incisions, skin tear, Ulceration, burns etc.
Type of wound: Skin tear
( Skin tear is a type of wound that range from a superficial to deep separation of skin layers. )
Ateology
Shear with a bladed weapon, traumatic accidents or friction( dry skin)
( Risk factors are blind or handicapped persons, dry skin due to dehydration and inadequate nutrtion, chronic medical conditions such as Diabetes mellitus, renal impairment or cardiac disorders and use of some medication that reduces skin thickness)
Nursing cosideration
Control bleeding( If it is a deep skin tear like lacerations or puncture wound; apply pressure on the site and hold the wounded site above the level of heart)
Assess the severity of the skin tear and need for stitches : Assess skin tear , wether it is a small or big
Cover the wound with sterile gauze and bandages
Check for infection around the skin tear site
Administer medication( analgesics) as per physician's order.
