Obtain nasopharyngeal swab to R/O Bronchiolitis and RSV and place in isolation until results confirmed. Begin IVF until optimal oral nutrition attained.

Explain Assessment and findings:

The family members/ care givers of the boy needs to be informed that the boy is experiencing severe symptoms of Bronchitis. He has increased heart rate and increased breath rate. breath sounds cannot clearly be heard due to crackles. the oxygen in blood is also low (only 85%, normal- 95-100% ) because the lungs are not able to take in the Oxygen due to severe lung congestion.nasal discharge is also causing obstruction in feeding due to which the child has not been fed properly and hence lost 10% of his weight in last 3 days(5.5 kg).

Plan of care/ Interventions:

Nursing care planning goals for a child with Bronchitis include maintenance of effective airway clearance, improved breathing pattern, relief of anxiety and fatigue, increased parental knowledge about the disease condition, and absence of complications.

Family members/ care givers of the patient will be explained about the following .

1 . The patient will be kept in isolation, until the result of the investigations are obtained.

2.Assessment of respiratory status, a minimum of every 2–4 hours or more often as indicated for a decreasing respiratory rate and episodes of apnea

3. Assessment of pulse rate and oxygen saturation using pulse oximetry

4. Provide humidified oxygen via face mask, hood, or tent.

5.Assess cough : onset, duration, frequency, mucus production, stuffy nose or nasal drainage.

6. IV fluids will be started until the required amount of nutrition is achieved.

7.Teach that the virus is transmitted by direct and indirect contact via the nose and eyes and that hands should be kept away from these areas.

8.Teach parents about the signs and symptoms of respiratory distress and infection, including fever, dyspnea, tachypnea, and expectoration of yellow/green sputum. This will encourages parents to seek prompt medical attention, as needed.

9.Teach of potential for spread of the virus to other family members and need for segregation of infant/small child from others.

10.Teach parents about the administration of medications prescribed.

11. Instruct parents on the importance of limiting the number of visitors and screening them for a recent illness. This will Prevent transmission of the disease to others; prevent further complications in the child with RSV
