Episodic care is health care provided to a client during an episode of illness. The main focus of this model of care is to treat the immediate issues of the client. This model of care is proven to have lot of risk factors as it lacks continuity of care and does not consider holistic care. UTi is the infectious condition in which patent require continuous care to prevent ascending infections of UTI.
Delirium is the condition in characterized by mental confusion and unaware if surrounding. So that the patient with delirium may not aware if his condition and he can't take care of himself. So that he require continuous care.
