Sexual assault or violence leads to not only physical injury or damage but also psychological and emotional consequences to the victim. While working with the client who has been sexually assaulted following areas should be assessed-

1. Physical examination and health assessment- the initial assessment should include physical health examination, assessment of injuries, wounds, damage to the physical health and any sort of trauma to body.

2. Psychological well-being assessment- sexual assault impacts the psychology and mental state of the individual, therefore the client with such an issue needs to be assessed for mental health consequences, symptoms of depression, trauma and PTSD.

3. Gynaecological assessment- the other crucial areas to be assess for sexual assault clients is they reproductive ability, system and any risk to unwanted pregnancies after the assault, this is necessary for the client and enables them to have options to make adequate decisions on timely basis.

Post-Traumatic Syndrome is the mental health issue that includes irritation, depression, anxiety etc. resulting due to some life trauma and events.

Short term nursing goals and interventions-

Assess the suicidal and homicidal ideation of client

Rationale- to ensure client's safety

Establish trust and rapport with the client by listening to the client and maintaining relationship

Rationale- this helps in achieving client's cooperation and enables to work in collaboration with the client

Long term goals and interventions

Encourage client to express emotions in safe environment with help of social activities and programs

Rationale- this helps client to be free to realize own feelings and repress their fears and overcome them.

Teach client to use visualization and imagery through relaxation and deep breathing exercises

Rationale- this helps in delivering better mental health approach, mind and body techniques and helps in reducing anxiety and past memories.
