The usual adult oral dosage of hydromorphone is 2.5 mg to 10 mg every 3_6 hours .

For IV Use , 1_1.5 mg every 3_4 hours slowly at least 2_3 minutes .

For dilution : 50 mg of lactose per ml can be used to dilute the hydromorphone .
