Developing discrepancy means helping clients identify conflicts between the problem behavior and their goals, values, and other behaviors. This is done through MI techniques such as double-sided reflections and in generally reflecting client's values, motivation and self-efficacy.

Patient - Centeredness is a core aim of the health care system,it focus on active patient engagement and provide importance to patient to findout their rights and their charecteristics,need,preference and circumstances..providing knowledge for their health improvement is important for patient so communication is the effective part between the patient and nurse..It is a complex and time consuming and its need more involvement from patient family and care giver..It helps providing decision-making power for patient and improves services and health outcomes...There is a barriers for patient engagement due to increased illness, growing aging population, health care costs, lack of technical support, lack of health care providers for patient engagement for their care..Medical practice gradually increasing focus on more collaboration,it also focus on managerial and economic consideration..Health care organization find patient engagement promises to deliver significant value for health care providers,patients and stakeholders..
Communication is most important skills when caring patient..listening is important when there is conversation between two people, it is a process of hearing..Listening will be under four catogories..Non-listener, marginal listener,evaluate listener,active listener..As a nurse we should be as active listener..it is a special communication skills will help to provide complete attention when patient talking to us by listening carefully their needs and words without interrupting..

Discrepency is a patient current behaviour that is differ from setting goals which can be motivated to make changes..To develop discrepency it is important to encourage patient to achieve their goals to make advantages to improve their health promotion..It should includes cost savings and health concern, and keeping the patient away from bad habits..Discrepency is the more valuable process to change patient behaviour,to develop this discrepency we have to create,emphasize and modify the patient mind a discrepency between the goal set and current behaviour..
