Rejection occurs due to simple errors, missing documents, missing patient insurance identification numbers. Claim rejection happens when a claim not meeting the patient data requirement they can not process the payment by payers. so these claims must be resubmitted after errors are been corrected.
This problem can be resolved by educating staff to improve patient quality data with accuracy, completeness, and consistency.
