Patient safety falling short in medical care and many hospitals found that harm to patients was common and number of incidence increasing over time..The main reason found for this slow improvement in patient safety is malpractice,drug errors and hospital- acquired infection.. landmark report in last 1999 found that medical mistakes cause almost 98,000deaths and more than one million injuries a year in the united states..Drug errors make hospitals stays more hazardous to patint health..

Most of the problems were temporary and treatable but some found very serious caused to death the study found..until there is a proper measurements to implement those strategies the progress in Patient safety will be very slow..
