Pathophysiology

The virus usually enters the body it starts to multiply in the lymph node. After this it enters the blood. Through circulatory system it reaches the liver and spleen .Innliver and spleen again the replication occurs called as secondary viremia ,This virus remains dormant in the sensory nerve ganglia and dorsal root.Reactivation of this leads to nerve damage and skin damage causing symptoms like blisters ,severe pain, fever, etc.

Assessment

  • A thorough history collection
  • Physical examination
  • Polymerase chain reaction test
  • An elevated level of IgM
  • VZV IgG avidity test

It is different from the symptoms of chickenpox in the following ways

  • It leads to cold sores and genital sores
  • No dermatomal pattern forms as in chickenpox
  • There won't be any specific symptoms
  • Pain ,dysphoria is common
  • Skin lesion ,erythema, pustule and ulcers occurs

Nursing management and client teaching

  • Follow infection control practice
  • Administer medication as per order(analgesics, antiviral, topical skin medication,steroid)
  • Monitor for complications
  • Health educate about the disease condition
  • Encourage patient to avoid intercoitis during active phase and during treatment
  • Encourage patient not to share clothing, towels
  • Complete the medication regimen as prescribed

The health promotion aspects for herpes zoster is to create awareness and encourage preventive measures.

The level of precautions needed in hospital are

  • Isolation techniques
  • PPE ,infection control practice
  • Disinfection of rooms and articles after patient discharge
  • Encouraging vaccination
