She has an lleostomy which she has managed on her own for several years Your Assessment Reveals the following findings Temp: 97.6, Pulse 98 bpm, Respirations 18/minute, BP 88/52 mm Hg, Pulse Ox 98% on RA (room air). Her lips are dry/cracked, skin is tenting, and you are unable to palpate peripheral pulses. Skin around stoma site is bright red and open in areas. Her ostomy pouch is found to be cut much larger than stoma site. She reports feeling very tired, and lacking energy.
