The RN is developing a plan of care for a 35-year-old female, newly diagnosed with breast cancer. Assessment findings include: the patient is teary and states, "I haven't been sleeping since the diagnosis." The patient doesn't make eye contact, has difficulty sitting still and her voice is trembling. Her mucous membranes are dry and she says she has not had an appetite. She says, "I don't know if I am going to need surgery to remove my breasts. I don't know how it will make me feel or look

1. The nursing diagnosis includes:

* Disturbed sleep pattern related to the hospitalisation and anxiety of surgery as evidenced by patients verbalization.

* Fluid volume deficit related to inadequate fluid intake as evidenced by dry mucus membranes and patients verbalization of loads of appetite.

* Stress and anxiety related to surgery as evidenced by patient questioning

* Fear of disturbed body image related to the surgery as evidenced by patients verbalisation

2. * patients maintains normal sleep pattern and demonstrates way to have good and sound sleep

* patient maintains fluid balance and reports good appetite

* patient demonstrates the coping strategies and relaxation techniques in reducing stress and anxiety

* patient improves her self-esteem and confidence and accepts the situation.

3. The diagnosis is the main priority because the patient is been for a surgery removes her one part of the beauty which she feels a s a asset for her life time. When the related symptoms are those which is present for patient can make her condition worse physically and psychologically she may come down hence making immediate screening and balancing the proper interventions at right time will reduce the risk factors and enhance the health.

4. The techniques of therapeutic communication here are:

* using silence were the patient is emotional and frustrated with the scenario

* accepting the patient firm and concern

* giving recognition by analysing the patient and providing a better compliment than an overt compliment

* touch which brings a best therapeutic psychological support to the patient

* active listening were by knowing the patient completelety

* making observation to know the patients mind set and chanfes

* offering hope and humor which makes the patient feel more comfort and sense of security
