0 Case Study : Parkinson Disease → Patient Demographics Initials edge: 78 years Biological Ser: Male Gender: Male • Race / ethnicity: Indian Allergies : No Idealth ldistory: History of current illness : client is presented with abnormal. rythmic movement of upper & lower Limbs since I months. It started on the distal part of both upper limbs Simultaneously. It was noticed more during rest that got aggravated during movements. Now the lower limbs were also affected & was not more on the both great toes. Patient also says & gives history of difficulty in walking in the form of difficulty in initiation & once he starts walking he bends forward with small & fast steps •However, he freezes during turning & has tendency to fall forward & backward. These is decreased Swinging of hands No Histooy of memory impairment, fall swallowing difficulty weakness of one half of the body.

2. Chief Com plains • de has complaints of - edb normal movements of upper & lower limb since I months -Difficulty in walking Since 4 months Past medical history. There is no history of - hypertension , cancer, diabetes, COPD, TB, asthma , injury, accident, surgical illness , psychiatriac disorder. Past Surgical history: Not significant • Social history de belongs to middle class family by occupation ho is farmer living in own house of one room & kitchen - socially active in gathering, doing Social work Exam Findings: •vital signs T - 98. 4°F P = 70 bpm R - 18 bpm B.P - 110/70 mm of Hg. • Pain : Not present • General appearance: Shuffling gait,

To thin body build, oriented , masked face. Major System finding : Inspection of eyes - Swelling of the eye lid Mouth - poor oral hygiene, missing teeth, dental caries. - Respiratory System Percussion :Risonant in left side &duellness in right side in right infra Scapular region.
