1. Home medication that probably precipitated Mr. West's G.I bleeding is coumadin . It is an anticoagulant and does not allows the blood to clot .

2. Serious complication of Mr.West's bleeding is Shock -Hypovolemic shock because when a person loses so much of the blood ,he or she wants into shock and affecting major organs perfusion .

3.Nurses note - on admission the patient was pale in look , sleepy but arousable on call and oriented . His vital signs were BP - 70bpm ( diastole blood pressure inaudible ) ,heart rate - 110bpm weak, and respiratory rate is 24 bpm . On examination ,S3and S4 sounds were present , peripheral pulses were present with mild edema . Lung sounds were clear and bowel sounds were present . Gastric tenderness was present and liver margin is 4cm below costal marine

4. Signs and symptoms of GI bleeding and fluid volume loss are :-

- dark red diarrhoea

- weakness

- pale in appearance

- tachycardia

- low blood pressure

- tachypnea

- disoriented

- sleepy /dizzy
