Older adult or Geriatric ( 65 years old or above):

Specialized assessment of each Geriatric patient should be completed on admission and the care plan should be as per the need of the patient.

The nursing plan of care for Geriatric clients should include

- Activities Daily Living

- Patient & Family Education to prevent falls

- Skin condition - care of skin and prevention of pressure ulcer

- Risk of aspiration

1. Care plan on prevention of falls

Assessment : The assessment includes the collection of data or the history of client. As the client is Geriatric , vulnerable to risk of falls.

subjective data: verbalized by client

Patient complains of dizziness, Weakness of limbs, secondary diagnosis, fracture of leg etc.

Objective data: assessed by client observation

Gait impairement, cast to limbs, weak in appearance, decreased strength, patients returning from surgical procedures with short term altered / decreased level of orientation as a result of anesthesia and narcotics.

Diagnosis:

Risk for falls related to (etiology / cause of problem) / impaired physical mobility.

Goals

client - centered goals : The needs of client includes activities of daily living, nutritional needs, toileting needs etc.

Interventions:

- Perform nursing assessment for fall risk upon admission and at least once in every 24 hours and document.

- All Geriatric patients are at high risk of fall. Apply fall prevention interventions based on High, Moderate and Low risk.

- Place yellow band on wrist and patient First card near the bed.

- Side rails always to be raised or up

- Lock all movable equipment before transferring the client.

- Adequate lightening is to be provided.

- Orientation to surroundings and routines.

- Use of call bells and call for assistance.

- Place articles within reach of patient.

- Position bed at lower level / add foot stool

- Teach patient to use grab bars.

- Educate patient and family about fall prevention.

- Help in toileting needs, bowel/ bladder assistance.

- Place assistive devices near the patient bed side

- Frequently monitor the patient - more rounds / visits.

- watch for side - effects of drugs.

- Instruct House keeping personnel to keep toilets dry and pathway clear.

- Obtain physiotherapy consultation.

Expected outcomes:
